Ventricular Assist Device Outcomes in Infants and Children With Stage 1 Single Ventricle Palliation

ACTION Learning Network investigators recently published Ventricular Assist Device Outcomes in Infants and Children With Stage 1 Single Ventricle Palliation in the November 2022 issue of the ASAIO Journal.

Examining outcomes in infants with stage 1 single ventricle (SV) palliation, this study is the first multicenter registry analysis focused on this challenging and understudied cohort of ventricular assist device (VAD) recipients. This study examined the largest pediatric cohort of 1 stage 1 SV patients to date, including 30 patients in the ACTION registry with a median age of 0.9 months undergoing VAD implantation between March 2018 and October 2020. Of these cases, pre-implant illness severity was high: 90% of this cohort required ventilator support, among other interventions. Researchers found that VAD implantation resulted in positive clinical outcomes–heart transplants or device weaning–in 63% of these cases. However, researchers also found that adverse effects of VAD support occurred in 80% of cases, including major infection, bleeding, neurologic dysfunction, renal dysfunction, and device malfunction. This study supports the clinical observation that this vulnerable subgroup will continue to be better served with transplantation, with VAD support as a bridge to this procedure.
